For Residency Application
Fulfill the following requirements:
2x2 and 1x1 pictures (2 each)
Curriculum vitae (include schools attended, rotations, hobbies, and interests)
Letter of intent addressed to Dr. Silvestre Gonzales (Chairman/Program Director) through Dr. Jose Nicanor Del Rosario III (Training Officer)
Letter of recommendation from department chair/training officer (including rank, in-service and hospital training evaluation and certificate of good standing or good moral character)
Certified true copy of the following: transcript of records including general weighted average, medical school diploma, certificate of internship, Professional Regulation Commission (PRC) board rating, PRC Board of Medicine certificate, and certificate of completion of at least one year in General Surgery
Photocopy of PRC license and certifications (e.g. seminars, trainings)
For lateral entry only: Must have finished at least 1 year of general surgery from an accredited training program of the Philippine College of Surgeons (PCS)

For Elective Internship Rotations
Medical students on their clinical years are welcome to choose SPMC Neurosurgery as their elective rotation which usually lasts 2-4 weeks. Rotating students will be exposed to varied neurosurgical cases both common and rare. They will be allowed to handle different neurosurgical patient under direct supervision of the residents. Emphasis is primarily placed on conducting a good neurological history and physical exam followed by a thorough clinico-pathologic correlation of the disease process. They are required to participate in doing patient rounds and attend endorsements, department conferences, and inter-departmental conferences if any.Â

Curriculum vitae (include schools attended, rotations, hobbies, and interests)
Certified true copy of your medical school General Weighted Average and Transcipt of Records in medical school (for 4-year medical schools, include a copy of your medical school diploma and transcript of records)

Applicants and students rotators who are not from Davao City have multiple residential options in the area surrounding SPMC. Due to the high volume of residents, interns, and students rotating in the hospital, living spaces have high turnaround times.
